The information you see in MyChart comes directly from your medical record at your provider's office. Your information is reviewed and updated after each visit.
If something looks wrong, ask your provider to update any incorrect information at your next visit. You can also contact the medical records department at your healthcare organization to request a correction to your chart.
Update the email address or phone number on your MyChart account in Personal Information. Contact your healthcare organization's help desk for help with changes you are not able to make yourself.
Your provider can see your full medical record so they can give you the best care. If both organizations participate in Care Everywhere, your provider might also see information from your record at another organization.
