You can create an account through your healthcare organization to access your information through MyChart. If your organization gave you an activation code on your After Visit Summary or in an email or text message, follow the instructions provided to create your MyChart account.
If you don't have an activation code:
Search for MyChart in the App Store or Google Play to download the app. If your organization has a branded app, search for it by name instead.
MyChart is only available at organizations that use Epic software. View the full list of MyChart organizations by selecting Find your MyChart. If your organization is not listed, contact them directly.
